What are the recommended cache settings?

What are the recommended cache settings whilst using this excellent addon?

Currently, I have cache on 'in all cases' The cookie notice however randomly appears on pages even though the 'Accept Cookies' button had been clicked early on another page on the site.

Turn off caching for this addon. Otherwise, it will show a notification regardless of the current cookies.
I think you can disable caching for block only.
