Does this add-on work with 5.7.5.13?

I noticed that 5.7.5.13 wasn't included on the list of compatible c5 versions and I wasn't sure if your list just wasn't updated or if a change in 5.7.5.13 made it incompatible?

We've just updated the add-on to version 1.0.4 to support version 5.7.5.10 -> 5.7.5.13 .
The module has been tested on version 5.7.5.13 and it works.

Because the module overrides the form core controller, we advise not to use this add-on on a website that is already live.

After installing the add-on I'm not able to submit the forms. For example, if you go tohttp://67.222.136.162/~outlooklife/... and try to submit the form that is on the homepage, nothing happens when I click the submit button. Any idea what might be going on with this?
