Form with multiple rows - not lining up side-by-side

I've set up a new form for our site and it has 2 rows. when i view the form on the site, they columns seem to alternate back and forth but never line up side-by-side. Is there a fix for this? Any help would be greatly appreciated. i've attached an image with the layout and what it looks like on the site.

Hi,

Seems there is an extra line in the first column.
Check to see if the second column has a custom class or something the like.
It could also be your theme that is messing up the columns.
