Permalink Browser Info Environment

I was just wondering if the multi step form can be created using addon?

if yes, is there any option where the next step depends on users input.

Also Does it have option to track the ip address of the person who submitted the form?

Sorry for the late reply!

The steps aren't depending on form input, but form fields or elements can be.
And yes, the form does record IP's if you want.
