I am experiencing multiple issues with this add-on. I have sent other requests in on the following issues of multiple dependencies of showing fields doesn't work after first instance, the inability to do an auto-respond email, and now the form functioning. For instance, when a required field is not filled out properly, rather than showing the error above the field as selected, it will go to a blank page and show this on screen:

In addition, once I was finally able to get a submission to take, rather than going to the selected URL, it goes to a blank page and the following is on screen:

When I was developing this form on this test page, I had caching turned off and Instant Page Speed Magic was not enabled. The errors were happening anyway. I only turned on caching and the other add-on after the errors to speed up the client's website awaiting your reply. That is not the source of the errors.
To double check and make sure, I completed uninstalled Instant Page Speed Magic and removed its directory, then cleared C5 cache, server cache and local browser cache and I am getting exact same errors. (1) The dependency rules are not working after the first instance of one on the form (2) Any errors of empty fields does not show the error code on screen, instead goes to a blank screen. (3) On successful submission, I am not re-directed to a thank you page that I set up, but rather get code on screen as before.
So basically, I am having exact same issues as before with the other add-on removed. I need to get this resolved quickly please.

You still have javascript issues.
Check your caching options in Concrete. Disable all caching, then remove the current cache and reload the page. See if that works.

Just tried that with no luck. Turned off caching on the website, cleared C5 cache, went to host and cleared hosting server cache, came back to C5 and cleared that cache again, went to browser and did "Empty Cache and Hard Reload" and I still get same errors and issues. I am stuck.
There are still javascript issues on the page.

You have a javascript on the page. Therefore Formidable can't be loaded.
You need to disable Instant Page Speed Magic. At least for this page.
