You're actually only able to define one "rest of the world" possibility.

But I'd see few options to solve your problem. First one is this:
- Create default flat-rate shipping methods e.g. "All over the world (Signed)" and "All over the world (Standard)"
- Then provide e.g. "Regional Shipping (within US and Canada)" as a third option in the list

Other option is simply to list all the countries in the world in the area list. This would then use the shipping rates that you define through the regional shipping add-on. It might be actually easier to do this straight to the DB through a script because the autofill options don't provide "all countries" as an option.

If you know like 5 countries that bring 80% of your customers, I'd probably just list those there and then default the others to the "Standard" shipping method (as the "Rest of the world" option).
